This disclosure is intended as a template to be used with the Paycheck Protection Program (PPP) application to inquire whether the applicant will be engaging an authorized representative (“agent”) to assist with the PPP application or refer the applicant to the bank for a PPP loan. The Bank should tailor this form to its needs based on current SBA and Treasury guidance. Consult with your Bank’s accounting professional and/or legal counsel if additional guidance is required.
