Regulation B C-4 Sample Notice of Action Taken, Statement of Reasons and Counteroffer Form is a combined counteroffer and adverse action notice. It is intended for use in notifying an applicant that adverse action has been taken on an application or account. Also, Form C-4 contains only section 615(a) of FCRA disclosure (that a creditor obtained information from a consumer reporting agency that was considered in the credit decision).
