Covered Borrower Identification Statement for Persons in Military Service

This form can be used for a covered borrower under SCRA to identify themselves.

WARNING – As of October 3, 2016, this form will no longer provide safe harbor for purposes of the Military Lending Act (MLA)–financial institutions either will need to run a check on the DoD’s website, or obtain verification of covered borrower status from a credit bureau, as required by 32 CFR ยง 232.5.
